Dr. Grace Park
Dr. Grace Park has served as the Program Medical Director for Home Health since 2010. As a practicing family physician, she is excited to be working with the Home Health team who provide care and support for clients that allow independent living in the home for as long as possible.
She brings a 'Generalist' perspective to the Fraser Health Program Management model of service delivery and enjoys working with other program leads to ensure smoother transitions for clients and seamless coordination of care within the health care system. She is actively involved with the Divisions of Family Practice in the work with the Integrated Health Network (IHN) and along with the IHN team making significant progress in Home Health integration with community GPs.
She is a strong proponent of primary care and was on the Provincial Working Group for GP4Me which aims to provide a primary care provider for all citizens of BC by 2015. She has been involved in Medical leadership since 1999 locally and regionally and is the recipient of the Peace Arch Hospital Medical Leadership award, the Friends of the Foundation award and the FH Above and Beyond award for work with the BreatheWELL development team.
She received her undergraduate and medical education at UBC and University of Calgary, MBA in Healthcare at UBC Sauder School of Business and is a Clinical Assistant Professor with the Department of Family Practice at UBC.
